CASE MANAGERS 

TO SERVE: Bamberg,  Berkeley, Charleston,  Dorchester,  Lexington,  Orangeburg, and Richland counties.


Case Managers conduct extensive assessments and develop case management plans based upon input from service participant,  family, caregivers and other service providers. Case Manager provide ongoing monitoring and service coordination of the participant's service plan.  Case Managers provide educational resources pertaining to abuse, neglect and exploitation; develop budgets for home and community based waiver services; assist with establishing and expanding social supports; advocate on behalf of and teach self-advocacy skills; respond to crisis; and organize multiple services through multiple agencies and organizations. Travel is expected at least 50% of time.

QUALIFICATIONS:
REQUIRED:  A bachelor’s degree from an accredited college or university, OR licensure from South Carolina Labor, Licensing and Regulation Board as a registered nurse AND at least (1) year of experience working with target populations in a health or human service organization. 


PREFERRED: A bachelor’s degree in psychology, sociology, human services, social work, nursing or  health administration, preferred.

Demonstrated ability to obtain Certification in Community Long Term Care case management standards as set forth by the South Carolina Department of Health and Human Services, REQUIRED. 


Certification in Case Management (CCM) or Certification in Brain Injury Specialty (CBIS) or ability to obtain certification  within one year of employment, strongly preferred.

Demonstrated ability to work with electronic health records.

Demonstrated ability to  maintain telecommuting performance standards.

Organized, detail-oriented, and ability to prioritize multiple tasks, projects, and meet deadlines with minimal oversight.

Background Checks: SLED and/or FBI; Child Abuse and Neglect Registry; Centers for Medicare and Medicaid Services; Nurse Registry (if applicable); Sex Offender Registry; Department of Motor Vehicle; PPD Screening.
