To empower individuals to embrace life through formal planning, education, advocacy, and using available community resources. Through services and supports Channel The Beacon, Inc. strives to bridge demographic, economic and social gaps by integrating services with defined efforts to minimize disparities, increase positive health status and increase access to healthcare, social services and affordable, safe, sanitary housing.
Care coordinators use evidence-based practices derived from the Chronic Care, Patient Centered Medical Home, and Human Service Models. These services assist individuals and their families through planning and accessing medical, social, educational, and other needed services in an effort to improve their overall quality of life, health status and promote independence. Care coordinators conduct extensive assessments and develop case management plans based upon input from individuals, their natural support systems and other health and human service professionals. Care coordinators provide ongoing monitorship and coordination for the provision of services included in the individual’s case management plan. Care coordinators provide educational resources pertaining to abuse, neglect and exploitation; develop budgets for home and community based waiver services; assist with establishing and expanding social supports; advocate on behalf of and teach self advocacy skills; respond to crisis; and organize multiple services through multiple agencies and organizations.
